Request Hemoglobinopathy Carrier Results

You can request carrier results from Newborn Screening Ontario (NSO) in the following ways:

1. Complete and submit a request form online, OR
2. Complete a downloaded request form [424.29 KB] and send it to NSO by mail, e-mail, or fax:

   • E-mail to newbornscreening@cheo.on.ca
   • Fax to (613) 738-0853

Please note: 

Carrier results are only available for babies born after November 24, 2006, as that is the date NSO began screening for hemoglobinopathies.

For the privacy and protection of the child, the request form must be completed by the child's health care provider or the child's mother (the mother is the only guardian known to NSO as her name is sent to us with the newborn screening sample). If another legal guardian is requesting these results, please mail the completed form with proof of guardianship [152.12 KB] to NSO.

Results will be sent to the child’s health care provider along with information sheets for the health care provider and parent/guardian.
